ACRT aims to enhance the health, independence and well being of adults living in the community with a neurological or physical disability.

Services available

ACRT works with clients to identify rehabilitation goals and offer tailor made programmes. Clients are seen in the most appropriate place for them e.g. at home, place of work or study.

Although intervention is not time limited, we will work with clients for around six sessions and review goals to determine progress, further intervention and future plans.

Some examples of our work:

• progressive neurological conditions such as MS, MND, Parkinsons Disease
• Acquired Brain Injury including stroke
• falls including balance group
• COPD including pulmonary rehab
• helping people to access community services such as leisure centres and voluntary organisation
• helping people make choices about their disability and coming to terms with the impact of their disability
• preventing admission to hospital
• helping clients function to improve following hospital admission
• a range of other groups such as Mindfulness and MS Support Group

ACRT also has a Consultant Allied Health Professional in Neurological Rehabilitation who provides assessment and makes recommendations for people who require specialist neurological rehabilitation packages in hospital or at home.

How to refer

• clients must be 18+ and be registered with a City & Hackney GP
• any health or social care professional can refer
• GP or Consultant referral may be required
• self-referral - clients already known to the service with progressive conditions such as MS can self refer
