Adult community nursing service
The Adult Community Nursing Service provides community-based nursing care, case management and advice to patients who are aged 18 years and over and primarily housebound.
Patients must have needs that require assessment and/or ongoing intervention by a trained nurse. Care may also be provided to ambulatory clients who require nursing care that is unable to be provided by practice nursing, and post-operative procedures like removal of sutures.
Our service
The service covers:
- District Nursing Service has eight teams, two in each of the four quadrants of the City and Hackney (NE, NW, SW, and SE localities)
- The Community Discharge Planning and Continuing Care Team (Long-term Care supports the discharge for people with long-term care and complex care needs
- Community Matrons provide a case management service for people with long term conditions, and work with the district nursing service
- Clinical Nurse Specialists in tissue viability, continence, heart failure, palliative care, respiratory care, Parkinsons’ disease, and Multiple Sclerosis
- First Response Duty Team (FRDT) is a multidisciplinary team comprising occupational therapists, physiotherapists, social workers and nurses. The aim of the team is to prevent hospital admission where appropriate and facilitate discharge from the acute sector for residents of City and Hackney. The nurses work closely in partnership with the Adult Community Nursing (ACN) service providing care to residents (age 18 years and over) of City & Hackney.
Aims of the service
The service aims to improve and/or maintain health, and promote independence by providing high quality nursing care to patients in the community setting. This is achieved by working in partnership with patients, their carers and other health and social care professionals to ensure that, wherever possible, care is maintained at home.
The adult community nursing service provides the following services:
- First contact/acute assessment, diagnosis, care, treatment and referral
- Co-ordination and management of care provided for patients with complex health needs
- Continuing Care, rehabilitation, long term condition management, and delivery of national service frameworks
- Assessment for nursing equipment
- Continence assessment and support
- Wound assessment and care
- Palliative care for the terminally ill, including bereavement support for carers
- Public health/health promotion and protection programmes that improve health and reduce health inequalities.

- Advice on promotion of continence
- Administration of medication
- Care to patients with long-term conditions which have complex needs
- Screening
- Immunisation
- Palliative and End-of-Life Care
- Assessment for equipment to promote and maintain independence
- Referrals to other health and social care services as necessary.
What you can expect
The District Nurses will assess your individual healthcare needs, discuss and plan your care, provide skilled nursing care in your own home and refer you to other agencies if needed.
The district nursing teams provide a seven-day service from 08:30hrs to 23:30hrs with a reduced service at weekends and out of hours.
