Structured education for type 2 diabetes
We run three different structured education sessions for people with type 2 diabetes.
Sessions are on Monday afternoons, 2pm-4pm, and consist of two sessions, one week apart. Referrals are welcomed for patients who only require education and do not need to see a diabetologist.
Patients who have never recieved education but have established diabetes are also welcome.
Group education in local community languages
We hold group education sessions on an ad-hoc basis for those who speak a language other than English. These groups are run by our diabetes lay educators, with the assistance of clinicians and interpreters. Groups are regularly run in several languages, including Turkish, Bengali, Gujerati, Vietnamese and Somali. Referrals should be sent to Pergul Kurt or Charlene Brade on the form below.
DESMOND
DESMOND is a structured education programme for people with type 2 diabetes. We have specialist training in running this programme. The programme consists of two, three hour sessions, with self-directed learning in between. DESMOND is offered to patients who attend the new patient clinic, or you can be referred directly by your GP or practice nurse. Referrals are taken for people who have had diabetes diagnosed for less than 12 months.
X-Pert
X-Pert is a 5-6 week programme for anyone with type 2 diabetes, no matter how long they have been diagnosed. The course is run at Homerton or at GP surgeries in English and Turkish. The course runs for 2½ hours per week for 5-6 weeks. People with type 2 diabetes should self refer for this course. All GP practices are notified of dates in advance, and have application forms. For the next available dates please contact Ann or Anna at Hackney Diabetes Centre on 020 8510 5920.
