The aims of the service are to help families;
- link services together
- provide a single point of contact in relation to a child with high care needs for family and professionals alike
- access the range of services available nationally and locally.
This is done through the allocation of a Key Worker who may be already known to the family in another capacity as one of their therapists or it may be a dedicated full time Key Worker. The Key Worker will work directly with the family to help them navigate through the range of services that could potentially be available to them.
How to refer to the Key Worker Team
Referrals are made to the Key Work Team via the MARS (Multi-agency Referral Meetings) that take place at the Hackney Ark.Referrals need to be sent to the Key Work Administrator on the above address either on the Hackney Ark multi-agency referral form or via a letter or assessment completed by a professional.
