Neighbourhoods Community Nursing Service

Neighbourhoods Community Nursing Service provides nursing care to housebound residents living in City and Hackney aged 18 years and over. Our aim is to promote and maintain healthy independent living at home for patients that can’t access their GP surgery or outpatient clinics.

We deliver a wide range of home nursing care including wound dressings, medication support, equipment provision, flu vaccination, end of life care and long term conditions management. We also provide leg ulcer clinics at Fountayne Road Health Center, Lower Clapton Group Practice and Bryning Unit at Homerton Hospital for mobile patients.

Teams within Neighbourhoods Community Nursing are made up of healthcare support workers, registered nurses, specialist practitioner district nurses and community matrons. The service works closely with GPs, Practice Nurses and other community services and specialists.

Who is this service for? We provide high quality care for people unable to access care outside of their own homes to promote independent living through information and training for patients, carers and families.

How to access this service: Referrals for housebound and clinic patients are accepted from healthcare professionals only. Patients being discharged from hospital that require housebound care will be referred directly to the service by the ward. Patients currently under our care can find contact instructions in their patient folder.

Opening hours are from 8:30am to 10pm each day including week ends and Bank Holidays.

After 18:00 please only call if your enquiry relates to a visit that evening, or you are currently under our care and need urgent support.

There is no overnight service patients requiring support during this time should call NHS 111 Out of Hours Service.
