Transitional Neurological Rehabilitation Unit

Homerton Transitional Neuro Rehabilitation Unit (HTNRU) is a seven bedded specialist neurological rehabilitation unit based within the Mary Seacole Nursing Home. 

Image of patient and therapistsThe service offers multi-disciplinary rehabilitation to patients who have a new neurological injury or have been diagnosed with a long-term neurological condition.

Patients may be admitted from other specialist rehabilitation facilities including stroke services or from an acute hospital. On occasion, patients are admitted from the community. The team consists of occupational therapists, speech and language therapists, physiotherapists, psychologists, nurses, rehabilitation support staff, a GP and consultant neurologist.

The team work together with patients to enable them to transition from in-patient healthcare to living within the community and live as independently as possible.

All patients will have a weekly timetable with individual therapy sessions and group activities. There is a focus on working on activities which will be done at home such as getting ready, meal preparation, managing a daily routine and going to the shops or groups.

HTNRU works in partnership with local community services to support the transition from being in hospital to living in the community.

HTNRU also form part of the wider pathway for adults in Hackney with neurological conditions and shares close links with the Regional Neurological Rehabilitation Unit (RNRU) based at Homerton Hospital.

 

Admission/ referral/ inclusion criteria

  • Registered with City and Hackney GP
  • Aged 16yrs and over
  • New neurological diagnosis or deterioration in long term neurological condition
  • Able to engage in intensive rehabilitation programme
  • Able to be managed in a community setting with GP medical cover
