Asthma

We provide an Asthma Service that supports those living with Asthma, who are either admitted with an acute asthma attack or specialist review to allow them to self manage their asthma long term.  This includes:

• Education to understand the condition and treatment
• Medication optimisation and review
• Assessing inhaled therapy and providing inhaler device training
• Development of self-management skills
• Providing individual Asthma Action Plans to enable patients to effectively manage their asthma
• Providing support to adjust medication as per the asthma action plan to prevent serious asthma attacks
• Providing a link between primary and secondary care
• Provide admission avoidance support via ACERS – separate link
